Top Ten Poster at ACC CV Summit

Intermountain Health presented results from our innovative approach to digital heart failure care delivery.

Jeff Goss, MSN, FNP, of Intermountain Health, presented a poster at the ACC CV Summit held in Washington, DC on February 1st and 2nd, 2024. The poster reviewed the results of the digital heart failure care delivery program that Intermountain Health implemented in order to improve utilization of guideline-directed medical therapy (GDMT) among heart failure patients. Story Health was selected as the virtual health partner for the program.

Key results include:

  • 6x average increase in GDMT from baseline
  • 85.7 Net Promoter Score (NPS)
  • 80% of total titrations were not associated with an in-person visit, saving an estimated 10 hours per month of clinic appointments
  • >10 point improvement in KCCQ (Kansas City Cardiomyopathy Questionnaire) score after 90 days on the program
  • 20% of patients showed improvement of EF to >35%, enough improvement that unnecessary CRT/ICD devices were not required. This improvement, plus other potential cost savings including risk reduction for heart failure related hospitalizations, corresponds to an estimated cost savings of $210,000 in this single area.
